If you have not had a Naturopathic consultation at Natremed before, you will need to download, print and complete the forms in the Welcome Pack. The Welcome Pack PDF document contains a medical history form, medication form, context of care questionnaire, a health appraisal form and a three day diet diary. You may need to print the the three day diet diary in landscape so it will fit properly on an A4 page. Once you have completed all the forms, you can mail them back to us, fax them at your convenience, or bring them along with you to your first consultation.
All new patients will need to download and complete this form. If you're seeing a Naturopath, please download the Welcome Pack which contains this form, and others required for first time Naturopathic consultations.
If you are seeing Julia Miles from Border Thermal Imaging, please download, print and complete these forms and bring them in with youfor your consultation.
If you are seeing a Naturopath, you will need to download form and choose three consecutive days to record what you're eating before your consultation. Bring the completed sheet in with you to help your Naturopath tailor advice specifically for your health needs. Be as honest as possible about all food and drink eaten to get the most from your consultation.
